Process and required documents to enrol kids for family 500+ program

500+ benefit is for residents having kids.

Offline (Office) Process:

Have the passport, PESEL doc, and Address Registration and if you have enrolled your kid on school, carry these docs to the office in person and physically go to the local office near Nowy Kleparz.

Online Process:

First, create a user ID in ZUS by using a trusted profile. Then upload Scanned copies of the visa (you and your kid), and the birth certificate of your kid, and it needs to translate into Polish.

You can apply it from the banking application as well. That is the easiest way, but it requires a birth certificate.

Initial Days in Poland

1. Rental Agreement -- Flat Owner will give.

7. Bus and Tram Card – Go to a nearby Bus stand to get the monthly Bus and Tram Pass. In Krakow “Galeria Krakowska” there is a centre near the Private Bus stand. Till your PIT 11 is generated, the monthly pass will be 148 PLN (for Zone 1 and all lines). Post PIT it will be 80 PLN.

For spouses, we can open a Bank account with the same features as the primary applicant.

Example:

1. Per month 300 PLN is the minimum expenditure from the spouse’s debit card. Else, the bank will charge 7 PLN as a debit card charge.

2. The same bank ATM is free withdrawal. For other Banks per month, one debit is free, rest 5 PLN per transaction is charged.

Winter jerkins and Spring Jerkins: In Poland, we should purchase two types of Jerkins.

As per my experience, I understood that in Poland +32 to -20 degrees temperature varied.

About NFZ clinic

1. Treatment is free of cost, visits and prescription will be available in your patient portal

2. NFZ visit might seem complicated in the beginning due to language barrier, the receptionists are not very welcoming(typical sarkari attitude) that's why most nfz will have low Google reviews by locals too

3. But once you are used to it and if you like the doctor, then it's super convenient

On 1st visit, you need to fill the registration form choosing your preferred doctor, for subsequent visits you need to book appointment in advance

4. I went there for general consulting for my 3yr old daughter, basically to verify the continuity of her vaccination and other vitamins

5. The pediatrician is not used to English but was very kind to us, nurse did the general height-weight checkup, eyes-tongue observation etc to see if there's any visible deficiency

6. The doctor checked our Indian vaccination chart, there were some differences in vaccine acronym, so she said she will need some time to study it and will provide a future vaccine schedule

7. Nurse gave us the next appointment after 10 days

8. My daughter had cough, so doctor checked and prescribed medicine. Good thing is you receive prescription online and also as a msg, so you can just tell the code and pesel to any Apteka and they will give you the medicine

In my case NFZ is in my society and just 5mins walk and we liked the pediatrician, so we prefer this than my Luxmed insurance. It's all about the doctor that you feel comfortable with be it NFZ or other private firm.
